You run the extractor.exe on the BIOS.FD file. It trims a header from the BIOS.FD and is suitable for recovery and/or BIOS chip programming.

    Code:
    InsydeFlashExtractor v0.2

    Usage: extractor INFILE OUTFILE

Ok, assuming you're using the InsydeImageExtracted 8.00MB BIOS.FD file...have you tried names like:

    Z5WE3 filenames
    Z5WE3.FD
    Z5WE3X.FD
    Z5WE3XX.FD
    Z5WE3XXX.FD

    Z5WT3 filenames
    Z5WT3.FD
    Z5WT3X.FD
    Z5WT3XX.FD
    Z5WT3XXX.FD

Looks like you will either have to purchase a BIOS chip programmer or purchase a new BIOS chip and solder it on.
If you do get a BIOS chip programmer, then make sure you backup your current "broken" chip first.
