If your Windows boot source (ISO) is legitimate, then either the installation media (DVD/PD) or the installation platform (HDD/SSD) is corrupted or contagion with bad sectors etc. Crosscheck & you'll find it out via own investigation. I'd suggest going with PD+SSD solution instead of DVD+HDD. P.S: Also, as you mentioned about 'slight modification', that also could be the reason, before going to check the h/w, ensure everything is working perfectly with an untouched, unmodified boot source (ISO)
