There's only two genders. Even trans people identify as the other gender. They don't make up a 3rd gender.

As unbelievable and unfortunate as it may seem there are people so far mentally gone that they can't even comprehend "gender" let alone identify with one. That could be an example of "Client doesn't know". Would it even be politically correct to use those terms on humans? Nevertheless it can and does happen through both military (Metallica's "One" music video comes to mind) and civilian scenerios. I remember an EMT I once knew who went to a car accident when he was still a probie and what happened there is a couple was driving late at night and the female decided to perform oral on the driver, well long story short (no pun intended) he orgasmed, lost control, and hit a tree and was instantly (to use your term) neutered. It kind of messed up the EMT because all he could do was put a diaper on the guy and ship him to the hospital. (I guess he finally pulled it together as last I knew he was a Fire Chief, but that was nearly 20 years ago.) That could be an example of "other" depending on how it affected the vicitim mentally.
