shortcut icon

    is it just the shortcut that is a problem? Maybe it just isn't seeing the ICON, right click customize add it from the programs EXE usually.

    have you tried rebuilding your icon cache

    This happens to me periodically. When it does, I just open the icon's Properties and change the icon. Sometimes I can simply re-choose the icon it was supposed to have and that fixes it.
