SFV patch consist of only just the updated patch file. It is just another way to update the ISO to the latest build, it just another alternative like UUP. If you are really concern about security then you should just stick to MVS or wait for techbench or MCT or Official ISO link and use W10UI or MSMG ToolKit or Nlite or other tools to integrate the patch. And by manually integrating the patch yourself with official source is still consider homebrew unless you are able to wait for M$ to release refresh build.
It's the same as 1809 for me. Same gaming performance, looks stable as well. Sandbox doesn't work, I've seen some article saying there might be required to add manual Defender exception for it but didn't get to test it yet. All good.
