Thanks for a wonderful tool. I like that it can be done without installing an app at all. Question, I managed to integrate the 5 recent updates into my LTSC directory. I saved a copy of it in my drive. If there is a subsequent Windows update, can I integrate the single update or do I have to start with all 6? Thanks!
Correct, the complete italian x86 ISO is not available. The link is an english iso which is used together with the executable file ( put them in the same folder and run the executable file) to create the complete italian iso.
What is the patch? This: SW_DVD5_WIN_ENT_LTSC_2019_32-bit_Italian_MLF_X21-96444.exe or this: 2019_LTSC_VLSC_17763.107_x86_SVFSFX.sha

In the end I succeeded, I put everything in a folder and I ran the SW_DVD5_WIN_ENT_LTSC_2019_32-bit_Italian_MLF_X21-96444.exe file and I managed to create the ISO in Italian, I tried the installation of the ISO in Italian, but during the installation it did not it gives me the possibility to choose the N version as for the 64Bit ISO.
Yes, the exe-files made by @Enthousiast are all 32-bit, to be able to run on either 32/64-bit Windows. What's on the ISO-file doesn't matter for the patch, it's bytes, period All questions aside, have you succeeded in creating an ISO and activated it yet?
