They are (usually) stored alongside in the same .msu as the GDR ones. When executing the .msu GDR gets installed. For LDR the .msu needs to be depacked and then installed. There are scripts like burf's update installer that do the job.
Since Net Framework v4.5.1 (available as a release preview) replaces v4.5, they are probably spending more effort on getting that ready for release than updating v4.5.
