If you want to call it a final ISO, on MVS Business ISOs it's 19042.508, by Business ESD it will be 19042.572. On UUP dump you can create your own Enterprise only ISO if desired.

An in-place upgrade is basically when you're installing the same version over the top of your existing installation. It's sometimes done to (try to) iron out kinks in the system, often as a last resort before just going for a clean install - in fact it's sometimes known as a 'repair' install. In the case of the 19042 build, the files are actually 19041 plus the enablement package that makes it pretend to be 19042, which is maybe why an in-place upgrade fails, because the system 'pretend' version is higher than the one you're trying to install.
